Safe Homes Form

     What is Safe Homes? The SAFE HOMES Program is a drug prevention program for parents/guardians of youth in 7-12th grades.  The program offers parents an opportunity to join together in providing safe environments for their teen to socialize while presenting a clear message to all youth that there will be NO USE of alcohol, tobacco or other drugs by youth in their homes.

 

2008 Pike County Community Needs Assessment

     Assessment of Pike County health care needs completed by Illinois Institute of Rural Affairs, Western Illinois University, as part of the HRSA Network Development Planning Grant.

 

Pike County Community Health Partnership Simplified Logic Model

     The 2008 HRSA Network Development Planning Grant enabled PCCHP to develop a logic model with evaluation component to guide the Partnership in fulfilling its mission and vision. A simplified version of the logic model is posted here.

Logic Model Training Presentation

     "Developing Logic Models to Plan, Communicate and Evaluate" presented to the PCCHP on Nov. 25, 2008 by Laurie Call, Illinois Public Health Institute.

Training objectives: 

  • understand the uses of logic models

  • identify basic components of a logic model

  • understand criteria used to assess which outcomes to measure

  • identify outcome indicators

  • indentify data sources for the outcome indicators selected
